SPORTS PHYSICALS

Dear Parent/Guardian/Athlete:

Athletic physicals are required for all students planning to participate in interscholastic sports.  The school physician, Dr. Weitzel, will be at the high school on June 21th to perform sports physicals for the 2013 Fall Sports Season.  The enclosed health&history form signed by the parent must be turned in prior to your sports physical or the school physician will not perform the sports physical.  At that time the athlete will be scheduled for his/her physical.

If you plan on taking your child to his/her private physician for a physical, he/she must return the completed Physical/Health and History Form to the nurse’s office by June 14, 2013.

In compliance with the State Education Department of School Health, the school physician must review and approve all sports physicals.  Parents and students must not contact the school physician.

BE PREPARED TO LEAVE A URINE SAMPLE AND EXPECT TO BE HERE AT LEAST ONE HOUR.
