Screening and Physical Referrals

During the yearly screening process, (vision, hearing and scoliosis) parents/guardians are notified if their child requires further evaluation by their own physician.

A written referral with school findings is mailed to the parent/guardian. On the back of the referral is a section for the physician to complete.

If your child has received a referral this year, and you have not yet returned the completed referral or notified the School Nurse of the results, please do so as soon as possible. Thank you.
